Anesthetized for wound care - see surgery report.
Discussed prognosis w/ SC and ME. I am concerned that the distal aspect of L digit 3 will have to be amputated - not sure about the long-term prognosis of L digit 4 distally. May lose talon - but hopefully will grow back.
Discussion is that prognosis is guarded, and unsure because he came in thin. But was he thin because of the mouth lesion? Looks like everything happened at the same time - and likely was trying to heal from all these wounds thus lost condition.
He is eating whole food now and getting "fat".

For now - continue to manage wounds. Will re-evaluate next week and see what status is of all wounds, re-visit whether or not amputation is in his best interest. Dr Gardner

Removed bandage on foot and evaluated. L digit 3 has deteriorated
since I last saw it - there is a bit "gap" and I feel that the
distal joint is a bit mobile. It doesn't look that clean either.

Scrubbed and flushed and opted to pack with Manuka honey again
to try to debride.
Also cleaned distal 4th digit. Opted to use Silver-gel to try
to get it to scab again. 

Moved to mouth - the scabbing actually looked better and smaller
to me today. Moved the tube tape to the upper beak to improve
access. Debrided scab w/in the mouth and under the beak. Uncovered
a very small defect that went through and through, but much smaller
than I would have thought. Much of the scabbing also came off
the outside of the beak, revealing some healthy-looking keratin
regrowing. Overall, good improvement. 
Applied Coloplast to beak/ mouth. Dr Gardner

Mouth and Left foot wound care. No ane used. Mouth's scab is
mostly adhered, both inside and out. Flushed with betadine solution
and flushed. Cotoplast applied. 

Left foot. Digit 3 has a large scab laterally that is pieced
together (not a solid piece). Lightly scrubbed and flushed. Added
cotoplast, telfa and wrapped with cast padding and vetwrap. Digit
4's small scab has come up and is exposing fresh tissue and the
base of the talon. The scab came off as we were lightly scrubbing.
Opted to put silver gel, hydrocolloid dressing and telfa on before
wrapping with cask padding and vet wrap. The 4th digit didn't
want to extend all the way but we didn't want to force it incase
the talon's keretin sheeth was damaged enough to come off.

Did bandage change and wound care awake. Patient is BAR.
Toe wound: removed bandage from L digit 3. The defect down to the bone has filled in well; there is exuberant pink granulation tissue overlying the bone and almost completely filling the defect. Toe flexes and extends well.
Lightly scrubbed and flushed wound and applied Coloplast, Telfa, padding, vet wrap.
The scab at the tip of digit 4 exfoliated revealing a shallow small wound. Cleaned and applied SSD.
There is a small superficial puncture on the medial aspect of the RIGHT hallux (likely from a talon). Cleaned quickly and also applied SSD.

Beak wound: The thick scab under the beak is partially lifting up. Removed and snipped it and applied Coloplast. Did not dare to explore further as the bird was not under anesthesia.
Also removed some of the plaque from inside the mouth.
P: Will explore this wound under GA next week - I think it is declaring itself. Most certainly the outside and inside wounds are connected and may need some type of closure.
Continue antibiotics. Dr Gardner

Bird is BAR. Bandage is over the middle of digit 3, but not covering the scab on the distal aspect of the toe.
Anesthetized with Isoflurane for radiographs, wound care.
RADS - no bony changes in toe noted. Jaw appears symmetrical although I don't have a good view of the lower jaw in the radiographs as it is superimposed over the upper jaw. No obvious bony changes there.

Toe is oozy, brought upstairs for wound care. The large scab on the lateral aspect exfoliated easily, leaving a fairly large, deep dirty wound down to bone. Cleaned well, then instilled Medi-honey to fill wound. Covered w/ Telfa, padding, vet wrap with distraction tabs.
Cleaned mouth and applied Coloplast - there are still some small plaques and the area under the chin is still dark and hardened. Will remain to be seen what will happen when this all consolidates - if the scabs come off when they are healed it may need more aggressive wound care.

Brought inside and anesthetized briefly to check toe and mouth.
Left digit 3 is bleeding a bit and oozing. Per Cas, the toe looks a bit more swollen. Palpated and cleaned. To me, it looks like the scab is cracking a bit where the toe bends and that is causing the oozing, possibly making it more inflamed.
Cleaned and applied SSD to toe.
Checked plaque in mouth - to me this looks like a traumatic plaque, there is also a scab and scarring externally. I suspect that the external and external wounds are related and might even be a through and through injury that is trying to heal.
Cleaned plaque with a little Betadine on a Q-tip. Some of the plaque loosened up and was able to remove. The flesh underneath looks good.
Not eating - may be painful.
P: Add Tramadol - if not improving in 4 days add meloxicam (didn't go straight to Meloxicam just because so skinny).
Clean mouth and foot EOD.
For now, Clavamox as antibiotic is fine.
If toe more swollen, repeat radiographs. If doing radiographs, add a DV of head (to look at R jawline). Dr Gardner

Veterinary Examination, 10/16/24:

Patient examined with manual restraint.

Oral cavity/beak: The upper beak near commissures has healing superficial abrasions just inside on upper palate. these lesions are not active lesions and are almost completely healed.
The lower beak has more significant lesions that appear to be previous trauma to the beak tissue because the lesions are near the commissure of the mouth on both sides, symmetrical but are superficial and are healing well.
There is no active drainage from any of the oral lesions and the lesions appear to be healing. I think this is most like previous traumatic injury rather than active infection. There is superficial, pale plaque like lesion just beneath the tongue as well that is healing.

Assessment: Previous, healing traumatic injury to the beak and mouth that appears to be healing. Recent infection cannot be fully ruled out however, and it would be recommended to have this bird on systemic antibiotic therapy for the next 10 days.

Chin: Area just beneath the lower beak has superficial scabbing and the dermis is healing well. There is no active discharge or drainage.

Body Condition: This bird was listed as emaciated at intake but today on exam there is pectoral muscle present and I would not classify this bird as emaciated body condition but rather moderately thin.

Treatment PLan: Add oral systemic antibiotic therapy to this bird's treatment plan.
Continue the anti-fungal therapy (terbinafine) as well. May need to assist feed this bird while the mouth continues to heal.
No need for topical therapy to the mouth for the lesions as the areas are healing well.

Antibiotic therapy to add to treatment plan: Clavamox Oral 10 day course.
